Nope, the prices listed include everything in the packages.  You can add optional products/services and those rates are also listed. The only other fee would be a travel fee if your wedding is a good distance from the tri-cities, but I am completely transparent about pricing and you'll know exactly how much the travel fee would be before booking.

Absolutely. The contract protects both of us. If for any reason I am unable to  fulfill my part of the contract, you will get a full refund.   (This has never happened) *knock on wood*

The sooner, the better, especially for May and October weddings.  Most of my couples book 3 to 6 monthss in  advance, but some book a year in advance and others are very last minute. My availability varies wildly.
